Medical Roster
The Medical Roster lists all members with medical coverage that are active as of the report date. The report lists members alphabetically by last name. Scroll right to view coverage information.
The Medical Roster includes the following columns:
- Coverage: The name of member's medical coverage
- Plan Name/Benefit Amount: enrolled coverage tier for member's medical coverage.
- Tier: enrolled tier of coverage for medical
- PCP/PDP: Displays the Primary Care Physician / Primary Dentist number
- Salary: The member's salary
- Per Pay Contribution: The employer's contribution amount for medical coverage each pay period
- Pre-Tax Per Pay: pre-tax amount deducted from member for medical coverage each pay period
- Post-Tax Per Pay: post-tax amount deducted from member for medical coverage each pay period
- Monthly Premium: monthly premium for medical plan
- Subsidy: whether member is receiving a subsidy for their medical plan
- Pre-Tax Deduction Code: pre-tax deduction payroll code
- Post-Tax Deduction Code: post-tax deduction payroll code
- Coverage Effective Date: when the member was enrolled in the medical plan
- Num Deps: number of dependents covered under member's medical plan
To run a Roster expand the Census Reports category and select desired Roster.
Enter appropriate Start Date and Run.
Open report by selecting the Download icon.
