The Beneficiaries Roster lists all members with at least one coverage allowing beneficiary designation and effective as of the report date. The report lists designated beneficiaries for each coverage on a separate row. If a beneficiary has not been designated for a coverage, it will be blank.  

The Beneficiaries Roster includes the following columns:

• Ben Relation: designated beneficiary's relationship to the member
• Beneficiary Name: name of designated beneficiary
• Ben Gender: gender of designated beneficiary
• Ben DOB: designated beneficiary's date of birth
• Ben SSN: designated beneficiary's Social Security Number
• Age as of [Specified Report Begin Date]: designated beneficiary's age as of the report date
• Beneficiary Level: indicates whether beneficiary is primary or contingent, and contingency level
• Beneficiary Type: indicates if designated beneficiary is a person, minor, or trust
• Percent: indicates percentage allocated to the designated beneficiary

To run a Roster expand the Census Reports category and select desired Roster.
Enter appropriate Start Date and Run.
Open report by selecting the Download icon.
